At the loss conference, I picked up a few brochures entitled:

Five Wishes

Here is a link and I thought it to be VERY helpful. It's a legal document in 40 states, and Wash. DC. You can order some on their website or call your local hospice or Ambercare.

Were they giving them away? On the site, they're $5.00 plus shipping.

Yes, they were giving them away. It was from our local Ambercare.com.

I looked at the pdf pamphlet version.

Such serious, heart-stopping decisions to ponder over. Would take time to figure out certain things.
